Abstract: Introducción. El comportamiento de la COVID-19, en relación con las comorbilidades es similar a nivel mundial; en estudios realizados en varios países se puede evidenciar que los pacientes de edad avanzada, con diagnóstico de hipertensión arterial, diabetes, obesidad, patologías pulmonares y renales subyacentes tienen como consecuencia mayores complicaciones con relación al tiempo de hospitalización, requerimiento de terapia intensiva y muerte. En Ecuador, en el 2022 se reportaron en las provincias de Pichincha 166 mil casos y 3197 fallecidos y Guayas con 65 mil y 2757 fallecidos, siendo las provincias con mayor impacto. Las principales comorbilidades en pacientes adultos mayores con COVID-19 son hipertensión arterial con el 49,7%, obesidad 43,3%, diabetes 28,3% y las enfermedades cardiovasculares 27,8%. Objetivo. Evaluar DMTII como factor de riesgo para desarrollar alteraciones renales y prognosis negativa en pacientes hospitalizados positivos para COVID-19. Métodos. El plan de desarrollo pretende estudiar a los pacientes hospitalizados en la unidad de salud IESS Ambato, en el período abril 2020 al treinta y uno de marzo 2021; en un estudio retrospectivo en el cual se incluyen pacientes adultos hospitalizados con prueba positiva para COVID mediante prueba PCR y además tengan como comorbilidad DMTII. Para el grupo control, se incluirá un grupo control de pacientes adultos hospitalizados con prueba positiva para COVID-19 sin diagnóstico de DMTII, procurando que los grupos etarios sean lo más homogéneos posibles al grupo de estudio. Los parámetros evaluados son nitrógeno ureico y creatinina, los mismos que serán tomados en el momento de ingreso del paciente. Las características de línea base entre grupos de pacientes con diabetes y sin diabetes se realizará la comparación con la prueba t bilateral para variables continuas y la prueba de Chi Cuadrada para variables categóricas. Resultados esperados. Con respecto al comportamiento de COVID-19 y DMTII; se espera poder determinar los grupos de edad de los pacientes y también si existe predominio en género masculino o femenino. Conocer si los pacientes con DMTII tienen un mayor riesgo de ser hospitalizados, requerir unidad de cuidados intensivos e incluso fallecer por mayores complicaciones asociadas. Determinar si los parámetros bioquímicos alterados, se relacionan con mayores complicaciones de los pacientes.
Description: Introduction. The behavior of COVID-19 concerning comorbidities is similar worldwide. Several studies worldwide show that elderly patients diagnosed with arterial hypertension, diabetes, obesity, and underlying pulmonary and renal pathologies have complications regarding the length of hospitalization, the need for intensive care, and death. In Ecuador, in 2022, 166 thousand cases and 3197 deaths were reported in the province of Pichincha, and 65 thousand and 2757 deaths in Guayas, representing the provinces with the most impact. The main comorbidities in older adult patients with COVID-19 are arterial hypertension with 49.7%, obesity with 43.3%, diabetes with 28.3%, and cardiovascular diseases with 27.8%. Objective. To evaluate DMTII as a risk factor for developing renal alterations and negative prognosis in positive COVID-19 hospitalized patients. Methods. The development plan aims to study patients hospitalized in the IESS Ambato health unit from April 2020 to March 2021. This retrospective study will include hospitalized adult patients with a positive test for COVID-19 and DMTII as comorbidity. A set of hospitalized adult patients with a positive test for COVID-19 without a diagnosis of DMTII will be included as the control group. IIt will be attempted to keep the age groups as homogeneous as possible, to the study group. The parameters evaluated will be urea nitrogen and creatinine, the same ones that will be taken at the time of admission of the patient. Baseline characteristics between groups of patients with diabetes and without diabetes will be compared using the two-sided t-test for continuous variables and the Chi-Square test for categorical variables. Expected outcomes. Regarding the behavior of COVID-19 and DMTII; it is expected to be able to determine the age groups of the patients and also if there is a male or female predominance. To know if patients with DMTII have a higher risk of being hospitalized, requiring an intensive care unit, and even death due to associated complications. To determine if altered biochemical parameters are related to greater complications in patients.
