Abstract: Background: Among 10-20% of individuals who acquire COVID-19, will require hospitalization in intensive care with the progression of Acute Respiratory Distress Syndrome (ARDS), a rise in serum ferritin has been identified as a risk factor in preceding studies as a clinical predictor of COVID-19 severity, values greater than 500 ng/ml are associated with increased severity of the septic shock, increased likelihood of progression of bilateral pulmonary infiltrate and a more severe prognosis. Objective: To assess the diagnostic performance of elevated ferritin values ≥ 500 ng/ml for mortality due to Acute Respiratory Syndrome in patients diagnosed with SARS CoV2. Materials and Methods: A retrospective cross-sectional analytical study was performed, with a population of 199 patients admitted to the ICU of the Hospital de Especialidades Eugenio Espejo, with the diagnosis of SARS CoV2 by RT-PCR (Real-Time Polymerase Chain Reaction) whose clinical data and variables of interest were obtained through the use of an anonymous and randomized database provided by the Hospital de Especialidades Eugenio Espejo, and approved by the Ethics Committee of Expedited Review of research COVID-19 of the MSP. Depending on the outcome, the data correspond to two groups (survivor and non-survivor). Results: Among the 199 patients, 107 (53.8%) died and no significant association was found (p=0.06) with ferritin values ≥ 500 ng/ml, the area under the curve (AUC) was 0.587 , with the best cut-off point being 902.8 ng/mL, with a sensitivity of 75.7% and a specificity of 40.2%, and a mortality predictive value of 56.4% (95% CI: 48, 6-63.8). The independent predictors of mortality obtained by multivariate logistic regression were arterial hypertension (OR=3.38; 95% CI=1.41-8.09; p=0.006), male gender (OR=2.51; 95% CI %=1.29-4.87; p=0.006), age ≥ 55 years (OR=2.71; 95% CI=1.42-5.18; p=0.002), the variables that showed a significant association with mortality adjusted to the other biomarkers evaluated was high D-dimer (OR= 2.272; 95% CI 1.063-4.856; p=0.034) and elevated Troponin T >14ng/L (OR=2.42; 95% CI=1.25- 4.67; p=0.008) and low lymphocytes <1x109/L (OR=3.08; 95% CI=1.10-8.61; p=0.032). The biomarkers that showed the best behavior of the Area Under the Curve (AUC) to determine mortality were the Neutrophil/Leukocyte Index (NLR) (AUC: 0.628; 95% CI: 0.504-0.752), NTproBNP (AUC: 0.642; 95% CI: 0.523- 0.762) and Troponin T (AUC: 0.660; 95% CI: 0.542-0.778). Conclusions: The increase of serum ferritin values does not represent an efficient clinical predictor of mortality in the studied sample. Arterial hypertension increased the probability of dying from COVID-19 by 3 times, regardless of the presence of DM2, cancer, obesity, CKD, alcohol and smoking habits, as well as ferritin levels. The increase of NLR, NTproBNP, and troponin T were the best predictors of mortality for the sample.
Description: Entre el 10 al 20 % de las personas que adquieren la enfermedad viral denominada COVID-19, requieren ser internados en terapias de cuidado intensivo con el desarrollo de Síndrome de Distrés Respiratorio Agudo (SDRA). Se ha evidenciado un aumento de ferritina sérica como factor de riesgo asociado estableciéndose en estudios precedentes como predictor clínico de gravedad de la COVID-19, por lo que se considera que valores superiores a 500 ng/ml se asocian con mayor severidad del choque séptico, mayor probabilidad de progresión del infiltrado pulmonar bilateral y un pronóstico más grave. Objetivo: Evaluar el desempeño diagnóstico de valores de ferritina elevados ≥ 500 ng/ml para la mortalidad por el Síndrome Respiratorio Agudo en pacientes con diagnóstico de SARS CoV2. Materiales y Métodos: Se realizó un estudio retrospectivo transversal analítico, cuya población estuvo integrada por 199 pacientes ingresados en la UTI del Hospital de Especialidades Eugenio Espejo, con diagnóstico de SARS CoV2 por RT-PCR (Reacción de la Cadena de Polimerasa en Tiempo Real) cuyos datos clínicos y variables de interés se obtuvo mediante el uso de una base de datos anonimizada y aleatorizada proporcionada por el Hospital de Especialidades Eugenio Espejo, aprobada por el Comité de Ética de Revisión Expedita de investigaciones COVID-19 del MSP. Los datos obtenidos correspondieron a dos grupos (sobreviviente y no sobreviviente) dependiendo del desenlace final. Resultados: Entre los 199 pacientes, 107(53,8%) fallecieron y no se encontró una asociación significativa (p=0,06) con valores de ferritina ≥ 500 ng/ml, el área bajo la curva (ABC) fue de 0,587, siendo el mejor punto de corte de 902,8 ng/mL, con una sensibilidad de 75,7% y una especificidad de 40,2%, y un valor predictivo de mortalidad del 56,4% (IC 95%: 48,6-63,8). Los predictores independientes de mortalidad obtenidos mediante la regresión logística multivariante fueron la hipertensión arterial (OR=3,38; IC 95%=1,41-8,09; p=0,006), sexo masculino (OR=2,51; IC 95%=1,29- 4,87; p=0,006), edad ≥ 55años (OR=2,71; IC 95%=1,42-5,18; p=0,002), Las variables que mostraron una asociación significativa con mortalidad ajustada a los demas biomarcadores evaluados fue el Dímero D alto (OR= 2,272; IC95% 1,063-4,856; p=0,034) y Troponina T elevada >14ng/L (OR=2,42; IC 95%=1,25-4,67; p=0,008) y Linfocitos bajos <1x109/L (OR=3,08; IC 95%=1,10-8,61; p=0,032). Los biomarcadores que mostraron mejor comportamiento del Área Bajo la Curva (ABC) para determinar mortalidad fueron el Índice Neutrófilos/leucocitos (NLR) (ABC: 0,628; IC95%: 0,504-0,752), NTproBNP (ABC: 0,642; IC95%: 0,523-0,762) y Troponina T (ABC: 0,660; IC95%: 0,542-0,778). Conclusiones: La estimación de los valores de ferritina plasmática no representa un predictor clínico eficiente de mortalidad en la muestra estudiada, l antecedente de hipertensión arterial incrementó 3 veces las probabilidades de fallecer por COVID-19, independientemente de la presencia de DM2, cáncer, obesidad, ERC, del hábito alcohólico y tabáquico, así como de los niveles de ferritina. El NLR, NTproBNP y troponina T fueron los mejores predictores de mortalidad para la muestra.
